Thank you for choosing Matthews Hematology Oncology Associates!  We are happy to serve you and your family.

We are looking forward to meeting you at your first appointment with us.  To allow our doctors and nurses to spend as much time with you as possible, please be sure to take care of the following prior to your visit:

-Complete the forms below and bring them with you to your appointment

-Be sure to bring your insurance card(s) & arrive 15 minutes before your appointment is scheduled

-Bring your medical records if they have not been faxed or mailed to our office

New Patient Forms

For your convenience and to reduce the wait time on your first visit, our new patient registration forms are below.  Simply click on each link to display the applicable form.  Print the form, fill it out and bring it with you to your first visit:

• New Patient Registration
• HIPAA Acknowledgement
• History and Physical
• Release of Medical Records
• Record of Disclosures
• Financial Policy
• Map to our Office-Matthews

In addition to the above forms, please bring the following items with you to your first and each subsequent appointment, if any changes are made:

• Insurance card(s)
• Driver's license or passport (Due to new Red Flag laws, we must have a copy of one of these legal documents on file)
• List of all current medications
• Co-pay money
• Medical records, if necessary
